Lead Form Feature

This article will explain how you can use the lead forms feature. It is an add-on feature in Shop Manager where you can add it on for just $29/month.


This allows you to create a lead form in Shop Manager and embed it on your website. When someone fills out the form, their information is automatically added to your Shop Manager account and sent to you via text and email.


  1. To enable the Lead Forms Feature, go to your Shop Manager account, then go to Settings > Subscription > then click 'Activate' on the 'Lead Forms Feature'

  1. Once it's activated, you can go to Settings -> Shops -> Your Shop Name -> Lead Forms
  2. Then click 'New Lead Form' on the top right

  1. Now you will be able to create the lead form. For the 'Form Name' and 'Campaign Name', it's best to put the name of the form and the website it will be on, for example:

  1. For the 'Redirect URL', you can enter the page you want to send the lead to after they fill out the form. Redirecting them to your Services or About Us page is a great way to get them to learn more about your shop.

As for the 'Host domain(s)' section, you'll need to put your website name only, for example 'aaronautomotive.com' or 'marysdetailing.com'. This section helps fight off spam.

  1. For the 'Theme' section, you can either choose Light or Dark depending on how well it goes with your website.

As for the 'Custom CSS' section, this is where you can add custom CSS to add different designs to the lead form. This part is optional.

  1. For the 'State Field visible' field, you can leave it off if you don't expect anyone out of state to visit your lead form.

As for the 'Zip field Visible' field, it's good to leave this one on so you can get a reading on where your clients are coming from.


  1. Then you can add custom fields to the lead form by clicking the '+Add Custom Field' button.
  2. In the 'Types' field, you can choose these different options: Text input, Text area, Select. When making the fields, the 'Name (internal)' section is where you can enter in a name that only you'll see. And in the 'Label' section you can enter the name of the section that will be shown to leads.

      1. Allows people to select an option from the picklist you create

  1. Then you can click 'Save' on the bottom right.
  2. Then you can go to your website editor and paste these two lines of code where you want the lead form to appear.
